The Agency’s mission is to provide professional and paraprofessional services to clients in their homes assisting them to achieve the highest level of potential in their day-to-day self-care activities. We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional’s point of view.
GOAL
Summit Skilled Nursing LLC is a skilled home care agency started by four registered nurses who recognized the need for high-quality, compassionate care delivered at home. We offer comprehensive services including skilled nursing, physical therapy, and occupational therapy.
Our fully trained team of licensed professionals supports clients with medication management, wound care, chronic condition and disease management, teaching on disease management, rehabilitation, post-hospital recovery, IV and respiratory treatments, infusion therapy, catheter care, and overall assistance with daily living.
